Duties & Responsibilities
1. Complete intake and needs assessment with families, which may involve a broad range of areas including housing, finance & budgeting, employment, education, food & nutrition, school & daycare, legal issues, health, parenting, and daily living skills
2. Interview and evaluate families to formulate Individualized Service Plans based on Program requirements
3. Work closely with families to identify and build upon strengths and develop strategies to address barriers and concerns identified through the assessment process
4. Help families build self-sufficiency plans
5. Assist families to transition to self-advocacy roles
6. Advocate on behalf of families and attend administrative hearings, if necessary
7. Perform home visits and/or unit inspections
8. Establish, evaluate, and monitor goals for families linked to measurable outcomes
9. Work collaboratively with interdisciplinary team of collateral providers and partners to ensure coordination of services
10. Maintain the structure and policies of the program through orientation of families by reviewing program rules, policies, and case management visits
11. Maintain accurate case records by documenting all services rendered including up to date case notes, telephone contact logs and referrals
12. Enter required data into the agency databases with reporting, as required
Requirements
Bachelor’s degree preferably in a Human Services related field (significant case management experience may substitute for degree)
Three to five years experience working with economically disadvantaged youth and/or families
Minimum of one-year assessment, advocacy and case management work in related field required
Experience working with populations with mental illness, development disabilities and/or substance abuse, and economically disadvantaged individuals and/or families
Knowledge, sKILLS, & competencies
• Knowledge and experience working with low income, culturally diverse, and at-risk families
• Demonstrated knowledge of community resources available to low-income individuals and families
• Excellent customer service, organizational, oral, and written communication skills
• Ability to manage multiple priorities and work in a fast-paced environment
• Proficient computer skills required in basic computer software and data collection systems
• Crisis management skills
• Ability to maintain accurate records and confidentiality of information
• Excellent attention to detail
• Bilingual (English/Spanish, English/Haitian Creole, English/Portuguese) preferred
